All accidents( including incidents and near misses) should be investigated. Accidents occur when
hazards escape detection during preventive measures or as the result of a combination of
circumstances that were difficult to foresee. A thorough accident investigation will help to identify
physical, process, environmental or administrative hazards which were previously overlooked. The
investigation will also reveal the need for new or more extensive safety training and safe work
procedures. The key focus of any accident investigation should be the determination of the facts
surrounding the incident and the lessons that could be learned to prevent future similar
occurrences which may lead to injuries or even loss of lives.
